We collect the information provided through enquiries, intake conversations and the referral portal, including contact details, information about the individual being referred and any documentation shared with us.
How we use information
Referral information is used only to assess suitability, plan support and communicate with the referring party, the individual and, where appropriate, guardians and professionals involved in their care.
Confidentiality
Referral and client information is treated as confidential. It is never published, sold or shared for marketing purposes, and access is restricted to authorized GoodHeart staff.
Storage and security
Information is transmitted over encrypted connections and stored with access controls, authentication requirements and audit logging appropriate to sensitive personal information.
Retention
Information is retained only as long as required for service delivery, legal and regulatory obligations, then securely disposed of.
Your rights
Individuals and their substitute decision-makers may request access to, or correction of, their personal information by contacting us.
